The NHS Sustainability Model is a predictive tool developed by the NHS for improvement projects. By answering questions about your project, the tool allows you to see the likelihood that your project will be sustained over the long term. The highest scores relate to senior clinical and leadership involvement in the project, as well as team participation. In other words, the more representative your project team of the people who will be working in the new and improved way, the more likely it is that your improvement will be sustained.
